This questionnaire will remain confidential and shall only be used for the purpose of evaluating a potential independent contractor freight agency relationship between CTS and applicant. This is not an application for employment for any groups affiliated with Covenant Transport, Inc.
NOTE: By submitting this Background Questionnaire, you acknowledge and agree that CTS can contact you for the purposes evaluating a potential independent contractor freight agency relationship with you, and that any such potential relationship shall be subject to such additional investigation, documentation and information gathering as is reasonably determined by CTS. You acknowledge that until a final written agreement is signed between you and CTS, no agency or other relationship is created between you and CTS, and this Background Questionnaire is offered by you solely for the purpose of initiating discussions between the parties, but creates no obligation on the part of either party to pursue or consummate any such relationship.
